Coup De Grâce - Utterly unnecessary; helpless targets take double damage; Translation: Visceral carnage in the language of love.

Is D:tD not deadly enough for you? Do you wish Sneak Attacks actually killed the enemies who were hit with them? Do your characters ever try to sleep in the face of danger? Check it out:

COUP DE GRÂCE
Half Action
Subtype: Attack, Melee, Ranged, Provokes
A character may only use this action against a target who cannot dodge or parry their attack. The character may use this action against targets who are unaware of them, characters who are unconscious, or have their back turned and are not aware that an attack is being made. This cannot be used against a target that is engaged in melee.
If using a ranged weapon to make this attack, the attack must be made from point blank range and gains no bonuses for range, however the attack does incur any penalties that may apply for the attack. If made with a melee or brawling weapon (including unarmed,) the target must be no more than 1 meter away.
The active character must make a Brawl, Weaponry, or Ballistics test with a -2k1 penalty against the Static Defense of the target. If successful, the active character rolls damage as normal, however this attack bypasses the target's Hit Points and halves the target's Resilience, rounding up.
Damage from a coup de grâce deals damage to either the target's head or gizzards, chosen at the time of the attack by the active character. A hero may spend a Hero Point to negate this attack.

Enemies that are unconscious are considered Helpless, and thus are always automatically hit for double damage.

I say that Coup set up you suggested seems a bit too deadly to be honest, overvaluing stealth for getting your murder on. A Wraith for instance could float underground underneath someone and use the action on them.

I think leaving it up to the Storyteller is probably best; there is actually some example of this in the Example of Play section in the first book:
Quote:

James Arroyo: "Uh, who's left?" James asks.
Jane Shepard: "You guys are. For the next two rounds. Start running." She does a terrible wolf howl
Rei Ayanami: ".....Did she drop the bag?"
James Arroyo: "I'll grab the bag and run like hell," James says.
Ai Yamashiro: "Angelus, who just stares between those surrounding him 'Mammon Nashim, what are you planning!? You will not get away with this!' 'Your security is busy hallucinating. It is over, and he casts a stunning spell on Angelus before blinking way like Eldarin usually do."
Rei Ayanami: "I run, then."
Ai Yamashiro passes Jane yet another mysterious note.
Anthony Hartman jumps at her howl, "You sure you're not actually a werewolf?" He looks at Ai. "Oh, and I run as well."
Jane Shepard: "If I was a werewolf you'd probably know by now."
Ai Yamashiro: "That was a nice touch."
Jane Shepard reads the note and smiles.
Ai Yamashiro: "You have a scared shitless dragonborn ripe for the, well, ripping."
Jane Shepard: "I'll go and kill him, since he seems to be holding his ground."
Ai Yamashiro: "Sure, want to describe? You sound like you're having fun." she smirks.
Jane Shepard laughs. "Traya bounds over, running almost on all fours, and leaps at him, driving him to the ground. She holds him down, even though he can't move, and her jaws slowly close like a vice full of teeth on his throat before twisting and ripping. A fountain of blood fills the air and coats her fur."
James Arroyo can only run his hair through his fingers in response to that.
Rei Ayanami looks a bit perturbed.
Ai Yamashiro nods.
Jane Shepard: "She howls as he dies under her massive form, then shifts back, still covered in blood. She runs a hand through her hair and sighs." Jane mimics this. "And looks at you all. ÂWell, at least dessert was good."
Anthony Hartman just smirks.
James Arroyo: "...Now

I had no idea about double damage, tbh. I can see how this would be unnecessary. I'm needlessly anal, and had a character try to "shoot the hostage" in the brain with a lasgun, but the damage just seemed minimal. But now that I know about that, I see that this measure is entirely unnecessary. I'd like to leave it up for posterity's sake, however. Someone may have use of it in the far-flung future. You never know.
