Titus 1:6 and 1 Tim 3:4 Paul says that the one of the qualifications
of eldership is that the elders' children must be faithful and
submissive. Does this mean that people are disqualified from eldership
or fulltime fivefold ordination (apostle, prophet, evangelist, pastor,
teacher) if his or her spouse and/or children are not believers, or
they are believers but not living a Christlike life ("dissipation or
insubordination" in Titus 1:6)?

Firstly, it must be made clear that Paul did not supply the ONLY
conditions for eldership; obviously, as there are people who meet the
qualification and yet are not fivefold. Since they are not the ONLY
conditions, they are also not the COMPLETE ones. What Paul was
speaking about was the IDEAL and PERFECT conditions for one to
function in the ministry. It is definite that when those conditions
ARE NOT MET, that the minister will have difficulties. However, since
the Scriptures must also be balanced with the OTHER revelation that
the gifts and callings of God ARE GIFTS and not rewards for character
or accomplishments, it stands to reason that there would be imperfect
situations where people called by the gift of God to a ministry,
through their own mistakes or lack of growth, fail in meeting Paul's
conditions. In such cases, having done the best they can in their
situations, they should be permitted to function in their gifts and
callings. The trial for them would be that they would be also carrying
the extra burden of having to constantly deal with their extra
imperfect situations which do not meet Paul's ideal conditions; and
they should keep seeking to meet those conditions but the free will
and free choice of others means that their responsibility cannot cover
for those free will choices of others. Note that Paul himself, as a
celibate, does not meet those conditions; I would thus assume that for
marriage counselling or counselling for bringing up children, Paul
would depend on others who are married.

What are the other passages on the qualification for eldership and
fivefolds besides those mentioned by Paul in the Pastoral Epistles of
1-2 Timothy and Titus?

The whole Bible. The best way to study them would be to study each
specifically is to study each ministry gift. The qualifications for
prophets, pastors (shepherds, priests) and teachers would be in both
the Old and the New Testament. The qualifications for apostles and
evangelists would be primarily found in the New Testament.
